POLICIES
Appointments – We value your time and aid to provide personalized attention during each appointment. To help us maintain this standard, please note the following:
For in-office appointments, please arrive 5-10 minutes early to complete the check-in and vitals process.
For virtual appointments, you must have a credit card on file. Any patient responsibility (copay, co-insurance, deductible, etc) will be charged once the appointment is underway.
If you need to cancel an appointment, kindly provide at least 24 hours’ notice. Appointments cancelled with less than 24 hours’ notice, or missed without notice (no-show), may incur a fee equivalent to the self-pay rate for the scheduled service. This fee reflects our commitment to reserving dedicated time for each patient. We understand that unforseen circumstances can arise. Fees may be waived at the provider’s discretion.
Prescriptions – To insure you have uninterrupted access to your medications, please allow up to three (3) business days for refill requests to be processed. Refills are not processed during weekends on holidays. Plan accordingly to avoid interruptions.
Refill requests must be submitted directly to our office via phone, email, or patient portal. We do not accept refill requests from pharmacies.
We encourage you to monitor your medication supply and schedule follow-up appointments as needed to manage your prescriptions effectively.
Emergencies – Your well-being is our priority. For non-urgent matters outside of regular office hours, you may leave a phone message or send us an email. All messages will be handled based on medical acuity and we strive to respond to all messages within 1 business day.
In case of emergency, please call 9-1-1 or visit your nearest emergency room. You may also contact the United Way Crisis Hotline at (904) 632-0600.
Please note: our office is not equipped to handle psychiatric or medical emergencies. If you are experiencing a crisis, please seek immediate assistance through the resources above.
