Lavern Dowell, APRN
Lavern is a double board-certified NP (Psychiatry and Family Practice) and a Certified Diabetes Educator. She accepts patients ages 8 and up. She is fluent in English and Spanish.

dr. Raul Soto-Acosta, MD
Dr. Soto-Acosta is a board-certified adult psychiatrist who takes a personalized approach to providing exceptional psychiatric care. He accepts patients 18 and up. He is fluent in both English and Spanish.

Carol Sickels,APRN
Carol is a board-certified Family Practice NP and adjunct professor. She is also currently pursuing certification as a psychiatric NP. She accepts patients 8 and up.

Charles Lorbeer, PhD, LCSW
Chuck is a Licensed Clinical Social Worker who provides counseling services to patients 18 and up, utilizing a variety of therapeutic modalities. He is also a Diplomate of the American Board of Forensic Counselors.

Victoria Payne, MS
Victoria is a Certified Addictions Professional who provides recovery- oriented substance use services, including assessment, treatment, individual and group counseling, and psychoeducation.

Michelle Grady Practice Manager
Michelle manages the practice and assists with Diet & Nutrition classes and coaching.

Hannah Dowell Medical Assistant
Hannah is a medical assistant and nursing student whose special interests include fitness and aesthetics.

Nichole Jackson Medical Assistant
Nichole is a medical assistant and nursing student who plans to pursue a career as a dermatology nurse practitioner.
PRICING and INFORMATION
Weight Loss Programs:
During your consultation, one of our providers will discuss your health history, goals, treatment options, and which plan may be best for your needs. All programs include monthly visits with a licensed medical practitioner, diet counseling, exercise recommendations, medication costs, and in-office injections (if applicable). At home injections are available as part of our Concierge Program (see program for more details).
Semaglutide Compound Program – $350 monthly. Semaglutide goes by brand names like Ozempic, Wegovy and Rybelsus. It works on the GLP-1 (Glucagon-like Peptide) receptors to reduce appetite and cravings. It is a once weekly injection with clinically proven weight loss results.
Tirzepatide Compound Program – $475 monthly. Known by the brand names Mounjaro and Zepbound, Tirzepatide works on both the GLP-1 and GIP (Gastric Inhibitory Peptide) receptors. This dual action not only reduces appetite and cravings, but also slows down digestion making you feel fuller longer. It is a once weekly injection that improves metabolic function for clinically proven enhanced weight loss results.
Alternative Weight Loss Program – $100 monthly. This program offers various compounded medication options that can be customized to fit your needs. We have both injectable and non-injectable product lines with a variety of proven weight loss medications.
Rates for Primary and Mental Health Care:
Self-pay rates:
New Patient Evaluation (60 minutes): MD = $300; APRN = $250; Therapist = $200
Follow-up Visits (15 minutes): MD = $100; APRN = $75; Therapist = N/A
Follow-up Visits (30 minutes): MD = $175; APRN = $150; Therapist = $100
Follow-up Visits / Psychotherapy Visits (60 minutes): MD = $250; APRN = $175; Therapist = $150
Insurance rates:
Insurance benefits vary by plan and can vary between medical and mental health care. We will verify your insurance benefits prior to your initial visit and let you know what you will be expected to pay at each visit. Verification of insurance does not guarantee payment of claims. Benefits are based on policy provisions and payment of policy premiums. If claims are denied, the patient/guarantor will be responsible for all balances accrued.
If you do not have insurance, or your insurance does not cover our services, the self-pay rates will apply. A good faith estimate is available upon request.
